Insulin Regimes

Basal Bolus Regime

A dose of long acting insulin (eg Glargine) is given at night delivering a 'basal' level of insulin (shown here by the red line).


Injections of rapid acting insulin (eg Novorapid) are given before each meal to mimic endogenous insulin secretion (shown here by the green line).



Mixed insulin regime

Patient requires 2 injections approximately 10 hours apart.


Their insulin injection contains a mixture of fast (green line) and slower-acting insulins (red line), eg. Mixtard 30 = 30% fast & 70% slower-acting

Teacher's Comment

You may want to think about the merits of the two systems and which patients are best suited to them. Things that you should consider include:

  • How close to normal physiology is this?
  • How good is control likely to be?
  • Can the Child or Young Person manage to do this? Is it too complicated or does a parent need to give the injections which cannot be given at school?
